The Hospice DPCS is responsible for the overall direction of hospice clinical services. Maintains the quality of patient care in accordance with the Agency’s objectives and policies, through planning, coordination, implementation, and evaluation of the hospice services provided.
Position Responsibilities
- Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel.
- Provide supervision and guidance to clinical staff to ensure the quality of services and maintenance of standards.
- Perform chart audits.
- Appraise clinical staff performance during their probationary period and annually, provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.
- Identify and evaluate patient needs and appropriateness of hospice by reviewing the initial intake screening form before assignment of direct care staff.
- Oversees the maintenance of patient clinical records and assures compliance with local, state and federal laws as well as Accrediting Body standards of care.
- Assist with the orientation of new staff.
- See patients in the home setting as needed.
- Participate in audits, IDG meetings and QAPI.
- Maintains adequate and appropriate Medical Supplies.
- Responsible for orientation of new personnel as well as in-services for current staff.
- Responsible for ensuring Medicare, State and Accrediting Body regulations are followed by all clinical staff.
- Assume responsibility for self-development by continually striving to improve his/her Nursing/Management practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.
Qualifications
- Minimum Registered Nurse licensed by the state, BSN or higher in nursing a plus. Graduate of an accredited School of Nursing.
- Minimum three years of DPCS experience in a Hospice Agency within the last five years
- Knowledge of business management, governmental regulations, and Accrediting Body standards
- Ability to communicate effectively both verbally and in writing
- Strong interpersonal skills and demonstrates self-confidence and positive attitude toward self and others.
- Ability to market and clearly communicate with customers and the community.
Job Type: Full-time
Pay: $9,000.00 – $11,000.00 per month
