Job Responsibilities:

The Marketing Liaison will be responsible for facilitating patient admissions via marketing and education activities with contracted and potentially contracted hospitals, skilled nursing facilities, respite centers, physician, and other community resources. This person will act as the point of contract for Hospital discharges and facilitate the admissions process for all patients. Some of the specific job duties include, but are not limited to:

  • Visiting hospitals on a daily basis
  • Creating working relationships with social workers/case managers at the hospital
  • Serve as a liaison with health care professionals (i.e. physicians, discharge planners, social workers, health care coordinators, etc.) and agency employees to coordinate safe transitions from medical facilities to the home setting/facility setting.
  • Screening patient clinical charts, verify payer source and discharge plan
  • Medication review
  • Marketing/social media
  • Provides Sub-Acute in-service programs to hospital and skilled nursing facility staff, physicians, community groups and others as directed.
  •  Works with Chief Business Development Officer and Director of Marketing to develop, establish, and maintain clinical services to contracted and potentially contracted hospitals, nursing facilities, area physicians, other agencies, and alternative care programs with potential linkage for LTC/SNF – Sub Acute care
  • Conducts all pre-discharge meetings, interviews, and meet and greets within designated accounts and assigned territories
  • Attend care plan meetings with hospital personnel on behalf of the agency to assure that the patients hospice needs are clearly identified and understood prior to discharge.
  • Inform hospital or other facilities personnel of case acceptance and document patient intake information (to include demographic, clinical, and payer information.)
  • Provide management with weekly and/or monthly updates on referrals and statuses utilizing various methods of reporting and analyzing data; make changes in plans as necessary.
  •  Assists with the development of annual goals and objectives

Requirements:

  • 2+ years health care marketing
  • Experience in the following industry a plus:
    HOME CARE, HOME HEALTH, SENIOR LIVING, HOSPICE
  • Established contacts in the community

Home Care

Responsibilities include:

  • Be responsible for basic care services such as bathing, grooming, feeding and assisting in the home for non-medical care and companionship.
  • Giving clients important social and emotional support.

 

The following are required for employment with Parentis Health:

  • Must be willing to demonstrate passion and ability to work with people with Alzheimer’s disease and other dementia-related diseases.
  • Certified Nursing Certificate desirable.
  • Ownership of reliable transportation desirable.
  • Clear background check including a BCI and FBI fingerprint screening.
  • Must be able to lift 25 lbs. or more.

 

Residential Care Facility-Verona Court

Responsibilities include:

  • Be responsible for basic care services such as bathing, grooming feeding and assisting in the home for non-medical care and companionship.
  • Giving clients important social and emotional support.

Home Health

The Registered Nurse Care Manager plans, organizes and directs home care services and is experienced in nursing. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and their families within their homes and communities.

Position Qualifications:

  1. Graduate of an accredited school of nursing.  One to two years of recent acute care experience in an institutional setting.
  2. Current licensure in State, CPR certification and valid driver’s license.
  3. Bachelor’s degree, with one year of home health care experience.
  4. Management experience not required.  Responsible for supervising home health aides.
  5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  6. Prolonged or considerable walking or standing.  Able to lift, position or transfer patients.  Able to lift supplies and equipment.  Considerable reaching, stooping, bending, kneeling or crouching.  Visual acuity and hearing to perform required nursing skills.

Job Description:

The physical therapist contracted or employed through Parentis Home Health is responsible for providing physical therapy services and for adherence to all conditions outlined in the care plan.

Position Qualifications

  1. Possesses a degree from a Bachelor’s or Master’s program in physical therapy approved by an accredited organization .
  2. Licensed to practice as a physical therapist within the State, if applicable.
  3. Community/home health experience is preferred.
  4. Demonstrates good verbal, written communication and organization skills.
  5. Possesses and maintains current CPR certification.

The Occupational Therapist whether contracted or employed through Parentis Health is responsible for the implementation of standards of care for occupational therapy services.

Position Qualifications

  1. Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
  2. Registered with the National Registration Examination of the American Occupational Therapy Association.
  3. Demonstrates good verbal, written communication, and organizational skills.

Title: Field LVN Full-time/Per Diem

Job Summary: Primary functions are to administer skilled nursing care, under the supervision of a registered nurse, for patients in their place of residence; and coordinate care with the interdisciplinary team, patient/family, and Parentis Home Health and Hospice staff.

Requirements:

1. Candidate must have graduated from an accredited school of professional nursing.

2. Current CA state license to practice as a Licensed Vocational Nurse in California.

3. A minimum of one (1) year home health experience is strongly preferred.

4. Must have an updated Physical and TB results.

5. Valid CPR Certification.

6. Nurse Liability Insurance (NSO)

7. Must have a current valid Driver’s License with automobile insurance in accordance with state and/or Agency requirements.

8. Works in patients home in various conditions; ability to work flexible schedule.

Required experience:

  • HOME HEALTH (GERIATIC) VISITS: 1 year

The Speech Therapist contracted or employed through the organization assess and treat persons with speech, language, voice, and fluency disorders. May select alternative communication systems and teach their use. May perform research related to speech and language problems.

Position Qualifications

  1. Possesses a degree which includes an internship from an approved Master’s program in Speech-Language Pathology, or as required by applicable law or regulation.
  2. Meets the education and experience requirements for a Certificate of Clinical Competence in Speech-Language Pathology, granted by the American Speech-Language-Hearing Association (ASHA).
  3. Is certified to practice Speech-Language Pathology.
  4. A minimum of one year’s clinical experience in Speech-Language Pathology.
  5. Demonstrates good verbal and written communication, and organization skills.
  6. Possesses and maintains current CPR certification.

Hospice

Plan, organize and coordinate hospice care in accordance to patient’s plan of care.

Position Qualifications

  1. Graduate of an accredited school of nursing.  One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.
  2. Current licensure in State and CPR certification.
  3. Bachelor’s degree, with one (1) year of hospice care experience preferred.
  4. Management experience not required.  Responsible for supervising hospice aides.
  5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

Position Qualifications

  1. A graduate of a bachelor’s program in social work accredited by the Council on
    Social Work Education.  Meets personnel qualifications stated in §418.114(b)(3).
  2. Minimum of one (1) year’s experience in health care, hospice experience preferred.   Understands hospice philosophy and issues of death/dying.
  3. Experience in a hospice care preferred.
  4. Demonstrates good verbal and written communication, and organization skills.
  5. Possesses and maintains current CPR Certification.
  6. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  7. Holistic, complementary and/or alternative modality training and experience preferred.

The hospice aide is responsible for providing personal care under the direction of a registered nurse.

Qualifications:

  1. Current Certification to practice as Home Health Aide in the state of California.
  2. One year experience as a CAN, hospice aid or home health aide.
  3. Must be a licensed driver with an insured automobile.
